Vision Therapy For Lazy Eye

Vision Therapy For Children And Young People

Advanced Treatment for Lazy Eye (Amblyopia) | Wow Vision Therapy

One common problem in children is they may have difficulty coordinating their eyes. To see something clearly, both eyes must be aimed correctly and focused at the right distance.

Any problems with aiming or focusing the eyes can cause a variety of symptoms, including:

  • intermittent double vision
  • headaches
  • eyestrain.

Children with these problems often do not complain about them, but may simply avoid tasks , which are difficult or cause discomfort.

An optometrist may suggest a program of vision therapy to improve eye coordination and focusing.

Surgical Lazy Eye Treatments

Cataract Surgery

In children, cataracts may be removed during eye surgery to treat a lazy eye. This allows for better development of vision in the weak eye. Cataract surgery for young and older children is carried out under general anesthesia.

Cataract surgery usually takes one to two hours. Your child may stay in the hospital overnight so the staff can check their recovery. Patients must use eye drops afterward.

Eye Muscle Surgery

The operation changes the position of the lazy eye by either strengthening or weakening its eye muscles.

As a result, the lazy eye appears better aligned with the strong eye. Eye muscle surgery doesnt improve the patients vision, but their eyes will look straighter. It also helps the eyes function better together.

Treatment Methods For Adults With Amblyopia

After a comprehensive assessment, a customized treatment plan will be created for the patients individual needs and visual skills. There are different methods for treating a lazy eye and often patients benefit from a combination of techniques:

  • Prescription glasses – are used to correct any refractive error, such as nearsightedness, farsightedness or astigmatism
  • Vision therapy – uses different exercises to strengthen how the brain works together with the eyes to optimize the entire visual system.
  • Eye patching – scientific studies prove that long hours of eye patching is no longer the effective method for treatment, but rather a combination of vision therapy and a few hours of patching is at least just as effective as full-time patching
  • Atropine eye drops – scientific research proves that daily administration of eye drops used to fog the vision from one eye, allowing the lazy eye to strengthen, is no longer the most effective method of treatment. Using the eye drops only on weekends will yield the same results as daily administration.
  • Virtual Reality – the FDA approved a virtual reality treatment method for amblyopia in which the patient watches modified movies and TV shows in order to strengthen the lazy eye using a virtual reality headset. Each eye receives its own image with different contrasts and learns to work optimally with the other eye and the brain in order to perceive the images properly

What Happens If Amblyopia Is Left Untreated

It is so important to understand that it is never too late to treat a lazy eye because the alternative of leaving this condition untreated can have serious consequences and lead to various conditions, such as:

  • Strabismus – this is when the eyes are crossed as opposed to being properly aligned
  • Central vision issues – an untreated lazy eye can cause central visual impairment
  • Blindness – permanent vision loss can be caused by an untreated lazy eye lazy eye is the most common cause of single vision loss in the U.S.

Lazy Eye Treatment For Adults

Lazy Eye Treatment

It was long thought that treating amblyopia, the medical term for lazy-eye, after the “critical age” of 8-12 years old was not possible. Unfortunately, many optometrists and ophthalmologists still believe, refer to, and share this outdated view. Research, clinical trials, and well-documented patient cases, now show that amblyopia can be treated well into adult hood. Many adults seek vision therapy to aid in their overall binocular function.

We now know that there is not a âcritical periodâ in which amblyopia must be treated. The ârecovery periodâ for these conditions continues through lifeâalbeit easier to treat if diagnosed and treated earlier. It is now known and supported widely by research from vision science, optometry, ophthalmology, and neuroscience, that patients of any age can successfully be treated for amblyopia . Again, despite these advancements, it is still best if these conditions are diagnosed and treated early on. You can read more about lazy eye in adults here.

Recent research has shown visual acuity as well as stereo acuity can improve at statistically-significant levels in amblyopic patients outside of the âcritical periodâ using virtual reality vision therapy technology. Another recent study also showed improved visual acuity and stereo acuity in amblyopic and strabismic patients aged 3-69 using virtual reality vision therapy technology who were previously unsuccessful in traditional patching or occlusion therapy.

New Research Shows That Eye Patching May Not Be The Best Treatment For Amblyopia And Lazy Eye

Author: Dr. Valerie Lam

Arggg Matey! You used to see those kids back in the day that had to wear a pirate patch over one eye and deal with the social pressures of having to walk around with an eye patched in order to treat their lazy eye.

An important idea to understand is that lazy eyes are NOT lazy!

Thankfully, recent research is suggesting that there is a better and even more effective way to treat amblyopia, without the use of the famous pirate patch. This new method is known as Binocular Vision Therapy.

Year Old Man With Lazy Eye Blinded In Good Eye

This gentleman came for advice after he lost the sight of his good eye leaving him to “cope” with his remaining lazy eye, only able to read the top letter on the chart. The hospital could not help and simply registered him as “partially sighted”. His previous occupation involved detailed figure work on an number of “work sites”, which as could not drive or read was no longer possible!

Just 2 weeks after starting vision therapy, he was able to read small print and after 3 months, his vision was up to driving standard!

A Lazy Eye Is Not At All Lazy Or Weak

A lazy eye is likely to be just as strong as the other eye, but because of impaired binocular vision, the signals from the amblyopic eye to the brain have been turned off.

It is important to note that a child with amblyopia can show no symptoms. It is therefore imperative to undergo functional eye exams to identify whether the child has or is at risk of developing amblyopia, particularly if a child is frequently squinting, or shutting one eye, or if problems arise in reading or maintaining focus during school or on the sports field. Amblyopia is treatable at any age, although the earlier the problem is found and treated, the more successful the outcome tends to be.

AmblyoPlay, a home-based vision therapy solution for children and adults with amblyopia , mild strabismus or convergence insufficiency , announced the launch of its software-based vision therapy solution in the U.S. market.

In the US, 100,000 children are born with conditions that require vision therapy each year. Colloquially known as lazy eye, amblyopia is a vision development disorder in which the vision of one eye is less developed than the other. Because the brain has a difficult time merging two images when one is of weaker quality, it starts to favor the stronger eye over the weaker one.

With AmblyoPlay, users perform vision therapy through red and blue glasses and interactive gaming software and are rewarded with tokens they can exchange for physical awards that are sent to their home. The programavailable via Windows and Mac computers, Android tablets and iPadsstimulates the eye muscles through two 15-minute sessions per day.

While it varies by location, vision therapy performed at private practices or vision therapy clinics can cost as much as $900 for an initial consultation and $170 per weekly appointment. Pricing for AmblyoPlay starts at $110 for a 3-month subscription and is available for purchase on AmblyoPlays website.

The Role Of Suppression In Amblyopia

Early concept think that suppression simply follows amblyopia. Treatments of amblyopia focus on occlusion or penalization of the better eye because the input from the amblyopic eye is weaker. The treatment does not concern suppression. The current concept thinks that suppression plays the causal role in amblyopia. Disruption of binocular function causes suppression leading to amblyopia.

Using dichoptic motion coherence threshold technique, quantitative measurement of interocular suppression is assessed in strabismic and anisometropic amblyopia. It is found that deeper suppression is associated with poorer vision in the amblyopic eye.

Due to limited success of conventional treatment for amblyopia and the new concept of brain plasticity, a variety of treatment strategies were investigated. These include dichoptic treatments and pharmacological therapy.

Can Lazy Eye Be Treated In Older Children Adolescents And Adults

FAQ About Kids Lazy Eye Vision Problem

The answer is yes! Sadly there is a common misconception, even amongst the scientific and medical community, that a lazy eye can only be treated up until around age eight. It is so unfortunate for people who are older than that to be withheld from having access to effective treatment due to this misconception that has been researched and proven to be incorrect. Treatment may take longer in an adult compared to a child, but people who are older than eight years old can still benefit greatly from successful results. Adolescents and adults have the advantage of generally being more compliant and determined to participate in the treatment plan as an older patient usually has a greater understanding of what is trying to be accomplished. Please allow yourself the opportunity to achieve optimal vision, no matter how old you are. We welcome you to schedule a consultation at our office.

Reduced Connectivity Between Brain Areas

Recent studies have shown that the physiological basis of amblyopia is mainly located at visual cortex and lateral geniculate nucleus. Functional magnetic resonance imaging study in human amblyopia suggests that V1 may be the earliest anatomic site in the visual pathway. Optimized voxel-based morphometry indicates that human amblyopes have reduced gray matter volume in visual cortical region. Amblyopic deficit not only involves circumscribed visual areas as visual cortex and lateral geniculate nucleus but also reduced the effective connectivity in different visual areas. The effective connectivity loss was found correlated with the degree of amblyopia. Feedforward and feedback connectivities are similarly affected.

Vision Therapy For Strabismus

Vision Therapy strabismus treatment without surgery with or without corrective lenses is the most effective and non-invasive treatment for Strabismus. In a Vision Therapy program, eye exercises, lenses, and/or other therapy activities are used to treat the brain and nervous system which control the eye muscles. Vision Therapy produces both positive cosmetic results and vision improvement .

Eye Patching For Lazy Eye

The medical eye patch is the oldest and most common treatment for amblyopia. The theory is that covering, or occluding, the strong eye with an eye patch forces the brain to use the weak eye. Having the brain depend on the weak eye encourages the development of better vision.

Kids often dread wearing it, and parents can have trouble forcing and monitoring their children to keep the patch on however, it has improved the vision of millions of patients with lazy eye for the last thousand years.

For many years, the assumption was that amblyopic patients are anatomically monocular and lacked any functional binocularity. However, recent findings have provided strong evidence for intact binocular processes in adult amblyopes that may have appeared to have been lost but were, in reality, suppressed.

Research has shown that applying monocular treatment methods does not reduce suppression, does not strengthen fusion, and does not help re-establish binocularity in amblyopic patients. In fact, stronger suppression may be associated with poorer response to patching therapy in children. Additionally, studies have shown that despite improvements in acuity in the amblyopic eye following patching treatment, there is a significant chance of regression and binocular outcomes remain poor.

Vision Therapy For Amblyopia

How Is Ambylopia (Lazy Eye) Treated In Vision Therapy?

Amblyopia or lazy eye is a neuro-developmental vision condition that begins in early childhood. It can also develop in people as a result of a traumatic brain injury or neurological condition. Those with amblyopia tend to have reduced eyesight in one eye, even when wearing corrective glasses or contacts. If left untreated, this condition can adversely impact a childs success in work, school, sports and social interactions, which in turn may affect their self-esteem and confidence.

Traditionally, patching the better-seeing eye was the only method used to treat amblyopia. However, this can be very uncomfortable for the child, has limited results past a certain age and does not develop the patients normal binocular vision or depth perception.

Vision therapy helps improve the childs visual abilities through a series of personalized exercises to enhance eye coordination, depth perception and reduce suppression. Suppression occurs where the brain inhibits the weaker eye altogether, leading to permanent vision loss.

Amblyopia Commonly Known As A Lazy Eye Is A Neuro

Lazy eye develops when one eye is unable to achieve normal visual acuity, causing blurry vision in the affected eye, even with corrective eyewear. The condition also commonly presents with poor depth perception and reading difficulties.

According to research, amblyopia affects up to 1 in 33 of the population this means up to 10 million people in the U.S. may have a lazy eye.

Amblyopia is the most prevalent neuro-developmental vision condition that affects the vision of up to three percent of the population. While the condition typically presents in early childhood, a lazy eye can develop later on in life as well.

Atropine. This is only effective if the sound eye is hyperopic. The atropine drop makes near vision blurry in the unaffected eye, forcing the child to rely on the amblyopic eye for near work. Atropine drops are an excellent alternative for patients who are noncompliant with eye patching therapy, as it is impossible to remove or peek around an eye drop once instilled. This treatment modality is excellent for high-energy patients who fuss with an eye patch, children with busy schedules or older patients with cosmetic concerns. This treatment option also promotes binocular vision. Drawbacks of atropine include being a prescription eye drop that needs to be filled and an intense stinging sensation on instillation. Light sensitivity is also an issue, as atropine produces a dilated pupil.

Bangerter foils. These can be applied to a spectacle lens of the sound eye to produce a blurred image at both distance and near. As vision improves in the amblyopic eye, practitioners can reduce the grade of blur by changing the foils. These also help promote binocularity and are excellent for older children with cosmetic concerns. However, a Bangerter foil should be used with caution on those suspected of noncompliance, as it could be quite easy to peek around the glasses.
