What is covered?

  • A free eye exam and vision test every calendar year.

Glasses and Replacements

Members Under Age 21:

  • Up to three pairs of glasses per calendar year. This includes replacing lenses on frame you already have.

Members Age 21 and Older:

  • One pair of glasses per calendar year. Lens replacements are not covered.

Medically Necessary Services

  • When medically necessary, additional services like corrective treatment, eye surgery and specialized eyewear may be covered.

Finding an Eye Doctor

Finding an eye doctor or participating facility is easy! With over 400 eye doctors to choose from, vision care should be just around the corner. Members can use our Find A Doc feature to find the best location.

Free Transportation

Louisiana Healthcare Connections members also receive free transportation to medical appointments. So, getting a ride to the eye doctor is easy. Members can , or call 1-855-369-3723 , Monday through Friday from 7 a.m. to 7 p.m.
