The following Services and materials are excluded from Coverage under the Policy:

  • Non-prescription items other than those listed in the Schedule of Covered Vision Services.
  • Services that the Covered Person, without cost, obtains from any governmental organization or program.
  • Services for which the Covered Person has been paid under Workers Compensation Law, or other similar employer liability law.
  • Any eye examination required by an employer as a condition of employment, by virtue of a labor agreement, a government body, or agency.
  • Medical or surgical treatment for eye disease, which requires the services of a Physician.
  • Replacement or repair of lenses and/or frames that have been lost or broken.
  • Optional Lens Extras not listed in the Schedule of Covered Vision Services.
  • Missed appointment charges.
  • Applicable sales tax charged on Services.
  • Services that are not specifically covered by the Policy.
  • Procedures that are considered to be Experimental, Investigational or Unproven. The fact that an Experimental, Investigational or Unproven Service, treatment, device or pharmacological regimen is the only available treatment for a particular condition will not result in Coverage if the procedure is considered to be Experimental, Investigational or Unproven in the treatment of that particular condition.
  • Any Vision Service rendered by the Policyholder.
  • Intraocular lenses.

    The UnitedHealthcare provider network includes more than 1.3 million doctors and other health care professionals and 6,500 hospitals and health care facilities nationwide. When it comes to UnitedHealthcare vision care providers, UHC Medicare plan members have access to local private practices and large retail chains like Walmart, Costco, Visionworks, Target Optical and more.

    In fact, UnitedHealthcare recently expanded its network of online and in-store vision providers to include LensCrafters, 1-800 Contacts and GlassesUSA. The UHC network now includes 96% of the nations top retailers, and 99% of UHC plan members in major markets have access to a network provider within two miles of their home.2

    You can use the provider search tool at www.uhc.com/find-a-doctor to search for eye doctors, ophthalmologists and other health care providers near you. You can also search for eye doctors near you by logging into your myuhc online account or by using the UnitedHealthcare app.

    Youll often hear the term in-network doctor when referencing health care and insurance. A UnitedHealthcare in-network doctor is one who accepts UnitedHealthcare insurance and agrees to the cost-sharing terms outlined in their contract with UnitedHealthcare.

    To get the most out of your vision plan benefits, youll need to know everything that your vision plan covers. UnitedHealthcare offers two vision plans. One plan covers glasses or contacts. The other plan covers both glasses and contacts.

    Do you have a “glasses or contacts” plan? You might think about switching to the vision plan that covers both glasses and contacts when your current vision plan ends.

    The good news is that you can reduce digital eye strain by wearing computer glasses designed to relieve focus stress from looking at computers, smartphones and tablets. Custom computer glasses contain anti-reflective coating and blue light filtering to ease digital eye strain.

    Patients Scrambling To Find New Health Care Providers Amid UnitedHealthcare, Montefiore Disputes

    UnitedHealthcare SCO is a Coordinated Care plan with a Medicare contract and a contract with the Commonwealth of Massachusetts Medicaid program. Enrollment in the plan depends on the plans contract renewal with Medicare. This plan is a voluntary program that is available to anyone 65 and older who qualifies for MassHealth Standard and Original Medicare. If you have MassHealth Standard, but you do not qualify for Original Medicare, you may still be eligible to enroll in our MassHealth Senior Care Option plan and receive all of your MassHealth benefits through our SCO program.
