The MCBS creates general categories of spending across services such as inpatient, outpatient, and medical provider . Use of dental services constitute one such service category. To calculate spending related to vision, and hearing services, we were able to identify use and spending related to optometrist services or to the purchase of eyeglasses and use and spending related to audiologist services or to the purchase or repair of hearing aids . All total costs are also separated by payerincluding private insurance, Medicare, Medicaid, and out-of-pocket spending.

Coverage of dental, vision, and hearing services was constructed from and limited by the information available through the MCBS for people in traditional Medicare. We were able to complement the self-reported information with MA plan benefit data for those enrolled in MA plans. The questions in the MCBS are limited to self-reports of dental and vision coverage. As there are no stand-alone plans available for hearing aids or services, the survey does not ask about them. To account for the coverage of hearing care that exists through some state Medicaid programs, we assumed that Medicare beneficiaries who were dually enrolled in Medicaid and lived in states that covered hearing care as a state benefit in 2016 had hearing coverage.29

The American Dental Association , however, prefers adding a new Medicare section, like Part E, for dental benefits. The ADA contends that a separate Medicare program is appropriate because dental care differs from other health services. The ADA also wants the coverage limited to beneficiaries with income up to 300% of the federal poverty level, a group that includes about 47% of seniors, the organization says. Sen. Joe Manchin also wants means testing for all of the proposed new Medicare benefits.

Some providers may wait to see how much Medicare will reimburse them for their services before signing on. The current bill doesn’t specify reimbursement amounts and leaves the decision to the Department of Health and Human Services. “I think everyone will jump on it if the reimbursement rates aren’t terrible,” says Geoffrey Joyce, director of health policy at the Leonard D. Schaeffer Center for Health Policy & Economic s at the University of Southern California in Los Angeles. “You can’t walk away from Medicare 70 million seniors.”

    Letting Medicare negotiate drug prices with pharmaceutical companies would help offset the costs from the expanded benefits. For the 2019 bill, the Congressional Budget Office estimated that negotiating prices would reduce federal spending by roughly $456 billion over a decade.

    Proponents say the change could also help lower prescription drug costs for beneficiaries. “There’s just a really strong appetite among the public for policy-makers to do something about prescription drug prices,” says Juliette Cubanski, deputy director for the Kaiser Family Foundation ‘s Medicare policy program.

    The current proposal from Democrats would allow Medicare to cap drug costs at 120% of the average price in six countries . But this would only happen if the government and the drug companies failed to agree on a lower price.

    The negotiation process would only apply to single-source brand name drugs with no generic competitors or roughly 25 drugs in 2024 and 50 in later years. These would be selected from the 125 medications with the highest net Medicare Part D spending and the 125 with the highest net spending nationally.

    Most medications will probably be unaffected, Cubanski says. Medicare doesn’t need to negotiate prices for every single drug because most are already inexpensive. Instead, the legislation is aimed at lowering costs for medications with no generic alternative. “They don’t have competition forces to keep that price in check,” she says.

    Medicare Would Cover Dental Vision And Hearing Under Senate Democrats’ Spending Plan

    The Senate Democrats’ proposal for a $3.5 trillion spending plan includes expanding Medicare to provide dental, vision, and hearing benefits. The proposal is now being negotiated in Congress.

    Currently Medicare does not offer much in the way of dental, vision, and hearing benefits. Medicare Part A will cover certain emergency or necessary procedures that are received in the hospital. For example, if you are hospitalized after an accident and require jaw reconstruction, Medicare Part A will pay for the dental work required as part of that procedure.

    Medicare Part B offers very limited coverage of some vision and hearing services. For example, while Medicare Part B wont cover routine eye exams, it does cover yearly glaucoma screenings for people at high risk and cataract surgery, among a few other limited exceptions. Part B will also cover some diagnostic hearing and balance exams if they are ordered by a doctor, but it will not cover routine hearing exams or hearing aids. There is no coverage at all for routine dental work.

    Many people choose Medicare Advantage plans, which are run by private insurers, instead of traditional Medicare because it is possible to get some dental, vision, and hearing benefits in most plans. According to the Kaiser Family Foundation, 79 percent of people in Medicare Advantage plans have vision coverage, 74 percent have dental coverage, and 72 percent have hearing aid coverage.

    Dental And Vision Services Are Not Usually Covered By Original Medicare

    The coverage for dental and vision is limited in Original Medicare. Because dental and vision services are not considered necessary medical procedures, theyre not fully covered by either Parts A or B. This means dental exams and supplies arent included in your Medicare coverage. However if youre in the hospital, a limited number of dental services may be covered under Part A hospital insurance.

    Eye exams, eyeglasses, and other vision services are not typically included as an Original Medicare benefit. However, Medicare Part B may provide coverage for a limited amount of vision services. For example, glaucoma screening, cataract surgery, testing for diabetic retinopathy, tests for macular degeneration, and diagnostic eye exams. Occasionally this may also cover prescription eyeglasses or lenses depending on your situation.
