Drugs For Visual Field Improvement

Some exploratory reports show that certain drugs might be able to improve visual fields. Though the studies were not randomized trials, they still offer some clues for future research: FS patients treated with calcium channel blockers or acetazolamide to increase blood flow who rapidly improved visual field function. Drugs that improved the visual fields of glaucoma patients had the following characteristics and limitations: the short-term changes were not related to IOP changes one could observe threshold shifts in relative scotomas , but very rarely though occasionally changes in the size of absolute scotomas, all the drugs that improved VF had in common that they improved blood flow in the eye , and improvements were more likely seen in FS+ but only infrequently in FS- cases .

Neurological Influences In Ocular Diseases

Loss Of Vision Treatment

Partial vision loss, with sometimes considerable residual vision is by far more prevalent than total vision loss. When vision loss develops in ocular diseases affecting the retina and optic nerve, this has repercussions at the level of brain processing as well. Though it is textbook knowledge that the retina is part of the brain, the complexity of the interaction between eye and brain is seldom considered and not such a simple matter. Here we need to ask on the one hand how much neuronal information reaches the brain, and on the other hand we also should learn how the brain processes incoming visual information by ignoring, enhancing or inhibiting it, and how visual cues are interpreted by emotions and cognition and how this impacts overall vision. On a systems level, the brain, by releasing stress hormones influence vascular tone, particularly in and around the optic nerve and thereby impair vascular autoregulation and perfusion of the whole body, including itself and that of the eyes . Furthermore, the brain is involved in other ways in eye pathology a partner in the intraocular / intracranial pressure balance which directly or indirectly impacts the consequences of eye pressure conditions . If any of these brain regulation mechanisms are altered, the brain-eye-vascular triad has a sensitive balancing act to manage which determines if, how, and when a vision problem is first diagnosed, whether it progresses, or whether it can be activated for recovery and restoration.

Regeneration Stem Cells And Retinal Implants

Protecting, replacing or regenerating cells to restore vision is currently a very active research field. However, adequately summarizing this basic research field is beyond the scope of this review and readers should refer to other sources . Briefly, clinical trials on axonal regeneration, though promising experimentally, have not been carried out due to the still too limited regeneration potential . Other approaches include the cell replacement and visual restoration by retinal tissue sheets aimed at increasing visual responsiveness to light by establishing synaptic connections between the transplant and the host . But stem cell transplantation was only tried in few patients and is not ready for wide scale clinical application .

Visual Problems In Neurological Diseases

As with other degenerative conditions, pathological changes have been observed at the level of the eye, subcortical visual pathways as well as visual cortical processing areas. Interestingly, Wostyn, Audenaert & De Deyn argue that AD also shares certain features with glaucoma in that both conditions lead to RGC death. Other neuropsychiatric disorders with visual system disturbance are psychosis/schizophrenia, autism, and dyslexia . Finally, neurodevelopmental condition such as cerebral palsy and Downs Syndrome also manifest characteristic visual disturbances. A study of 120 children with cerebral palsy found that 50% had strabismus and/or significant refractive error and 11% had visual field defects . Crucially, children with cerebral palsy are often affected by unique visual/behavioural dysfunctions, e.g. visuo-spatial and motion processing, complexity/crowding, and attention deficits .

What Our Patients Are Saying

Sehschrfe Augendruck Und Refraktion

Is Alternating Current Stimulation For Treatment Of Optic Neuropathy Real Science Too Early To Tell Or Quackery

I think this is more suited to AskScienceDiscussion than AskScience.

A friend of mine has found this clinic in Berlin called the Center for Vision Restoration, run by a Dr Anton Fedorov. She is hoping that they will be able to help her regain some of her vision. From an initial look I’m very concerned about her pinning hopes on this and spending a lot of money on it.

I can see on Google Scholar that Dr Fedorov has published a number of papers on the subject, although I’ve not got access on those sites to read the full papers. I can also see from Google Scholar that the papers have been cited multiple times.

What should I be looking for to tell whether there is real science behind the claims? It appears they have run clinical trials on it but surely there is a conflict of interest when he is selling the procedure and it’s his company running the trials?

Would appreciate any advice.

Repetitive Transorbital Alternating Current Stimulation

After BASELINE assessments patients were stimulated with either rtACS or sham-stimulation on 2 × 5 weekdays for 25 min at day1 to 50 min at day10 with eyes closed. Four 10mm Grass gold electrodes , 2 for each eye, were placed on the skin near the orbital cavity , and biphasic square-pulses were applied in bursts . The return electrode was positioned on the right arm. Stimulation current strength was 125% of phosphene threshold recorded during 5 Hz stimulation. rtACS was conducted with frequencies between 825 Hz. To conceal group identity, sham-patients received only one ACS burst per minute to create phosphene experiences.

Brain Degeneration In Ocular Disease

Nervous system degeneration in aging can affect the structure and function anywhere along the visual eye-to-brain axis . Obviously, what the brain perceives is a function of how much retinal input it receives. But if the brain is also affected by any additional loss of neurons, loss of functional connections in down-stream visual structures, or vascular autoregulation problems, then the complexity of the eye problem is far greater . This is the case when, for example, general cell loss occurs during normal aging with specific vulnerabilities of the brains visual system structures . Even in traditionally ocular diseases , a selective degeneration of cells in visual brain nuclei has been shown as indicated by atrophy of the LGN, optic radiation and visual cortex , and even non-visual brain structures that control emotions .

Center For Vision Restorationberlin

Centre for Vision Restoration was founded to deal with disorders of vision and aims to help visually impaired patients to regain or improve their eyesight. The problem of vision restoration is not just a medical issue, but involves many other levels – psychology of low vision, sociological integration and adaptation to day-to-day activity. Our Restore Vision Approach dates back to 1993 when Dr Fedorov began his work as a neurologist and researcher at the Human Brain Institute . For 20 years we have helped a considerable number of vision impaired patients and have gained invaluable experience. Our work involves an interdisciplinary approach, combining neuro-ophthalmology, neurophysiology, modern ophthalmology examinations and neurology. Improvement of quality of life which could be achieved by applying only individual care with a combination of different medical approaches is our primary goal. We learned that vision impairment is very serious condition, affecting many areas of everyday life. Recovery can be difficult and challenging, it requires dedication, persistence and the ability to adhere to the prescribed treatment. But the journey is well worth it. Based on our deep comprehension of the vision restoration method and continuous contact with our patients, we strive to attain exceptional results.

Treatment Outcome Prediction Model

In order to analyze which areas of the visual field respond with improvements after rtACS, we separately analyzed different visual field states based on HRP measurements and developed a treatment outcome prediction model to predict from the BASELINE results of HRP visual fields the extent of vision restoration . To this end, different visual field states were determined by superimposing repeated HRP measures to determine which visual field regions are intact, partially damaged and absolutely impaired .

The difference between groups in the primary outcome measure was found to be maily caused by a reduction of the absolute scotoma region in the rtACS-group. The size of the absolute defect significantly decreased after rtACS when compared to sham-treated patients . Post-intervention changes in the intact and residual visual field did not differ between groups.

Out of a total of 12 features deemed relevant based on previous studies , two features of HRP visual fields at baseline were associated with visual field improvements after rtACS: greater neighborhood activity, i.e., the mean detection rate of all HRP test positions within a 5° radius around each detected location, and greater residual function, i.e. the detection rate at a given visual field position . This suggests that improvements of residual vision are the key factor of vision restoration.

Sample Size And Randomization

The sample size calculation was based on results of two earlier pilot studies at the Institute of Medical Psychology considering similar patients and equivalent stimulation schemes, i.e., rtACS- and sham-groups as in the present trial. In the first trial a mean percentage increase of 65.61 ± 104.35 was observed in the rtACS-group and 16.93 ± 31.22 in the sham-group . The corresponding results of the subsequent trial were 30.68 ± 41.97 in the stimulation group and 9.57 ± 12.05 for the sham group. Using these means and standard deviations in a sample size calculation for a two-sample t-test with nQuery Advisor 7.0 resulted in 41 patients per group for the first trial and 36 patients per group for the second trial. Therefore, we adopted a conservative approach of considering up to 10 percent drop-out and planned to recruit 45 patients per group. Patients were assigned to either the rtACS- or the sham-group by Randomization In Treatment Arms software with stratified block randomization considering the study center and the VF defect depth at BASELINE as potential prognostic factors. High vs. low defect depth was defined as stimulus detection rates below vs. above 30% inside the VF defect.

Randomization resulted in comparable average baseline situation and demographics in the rtACS- and sham-group. Only the percentage of males vs. females was unequally distributed but this was independent of study results.
