Growing older and needing reading glasses have often gone hand in hand, but it might not need to anymore.

In October, the US Food and Drug Administration approved Vuity, an eye drop used to treat presbyopia, or age-related blurry near vision. In early December, the drops became available at pharmacies with a prescription for people with age-related blurry vision the first medication of its kind to treat the condition.

Dr. Christopher Starr, an ophthalmologist and spokesperson for the American Academy of Ophthalmology, said in an email he’s prescribed them to patients and has also been using them himself.

“They work,” Starr said.

How Was Vuity Developed

The story of how these eyedrops were developed goes back to a side effect of pilocarpine when used as a glaucoma eyedrop. Patients using the eyedrop experienced the side effect of constriction of their pupils and increased ciliary body tone.

The ciliary body is located behind the iris and is a ring-shaped muscle that changes the shape of the lens when the eye focuses. The ciliary body also produces the clear fluid that fills the space between the cornea and the iris.

When the tone of the ciliary body is increased and the pupils constricted, near vision is clearer. The new FDA-approved eye drops for presbyopia are a low-dose of pilocarpine with a proprietary pH technology that causes the eyedrops to rapidly adjust to the pH of tear film. An eyedrop that causes pupil constriction is called a miotic.

What Are The Side Effects Of Using Pilocarpine Eye Drops To Correct Close

“What does constriction of the pupil do?” asks Dr. Pandit. “It keeps your eye muscles active and closes off some of your peripheral vision, so eye strain and tunnel vision may result for some people.”

The most common side effects reported while using pilocarpine prescription eye drops are:

  • Increased tear production

The drops may also impair your vision in low-light situations, so they’re not recommended for night driving.

“There will eventually be other drops available for improving near vision that work in slightly different ways or with slightly different formulations, which may help overcome some of the unwanted side effects of the pilocarpine drops,” Dr. Pandit adds.

How Do Vuity Eye Drops Work

VUITY takes advantage of the pupils natural ability to resize by using a pilocarpine formulation as its active ingredient. Pilocarpine is one of the oldest drugs used in ophthalmology.

Its been in use for decades , mostly to treat glaucoma induced by damage to the optic nerve, said Dr. Stephen Orlin, an ophthalmologist at the University of Pennsylvanias Perelman School of Medicine.

Allergan has a formula that mixes pilocarpine with the companys proprietary pHast technology to deliver clearer up-close vision. VUITY creates a pinhole effect on the eyes by constricting the size of the pupils. The smaller pupil creates a reduction of the peripheral light allowed to pass through them, making objects appear sharper.

The drops treat mild to intermediate presbyopia, but because they work for less than half a day, they cant serve as a permanent replacement for reading glasses. They will reduce the amount of time you need to wear the glasses you use for reading.

VUITY does not treat other eye conditions such as astigmatism, nearsightedness and farsightedness. People with existing eye defects will need to hang onto their prescribed eyewear for corrections and may not benefit much from VUITY.

The drops are meant for mild to intermediate cases and wont be as effective for people aged 65 and older, said George Waring, the principal investigator of a VUITY clinical trial.

Side Effects And Other Considerations

In the clinical trial, > 5% of patients reported headache and eye redness. 1-5% of patients reported blurred vision, eye pain and watery eyes. Patients taking Vuity are advised to be cautious when driving at night and doing hazardous activities in poor illumination. Rare cases of retinal detachment have been reported in patients using other miotics, so patients using Vuity are advised to seek immediate medical care if they experience sudden onset of vision loss.

How Effective Is Vuity

In two clinical studies, 750 people with presbyopia were assessed to determine the efficacy of Vuity eye drops. Half the participants were given Vuity to treat their blurred vision, and the other half received a placebo eye drop.

After a 30-day trial, those who received Vuity showed a significant improvement in near vision clarity, especially in low-light conditions. The studies also found that Vuity had no impact on distance vision clarity.

During the three-month trial, Vuity worked best in people aged 40-55, and side effects included headaches and red eyes.

Where Can I Find Vuity Eye Drops

VUITY is available only by prescription. Since the drugs approval in October, its parent company Allergan has made it available in pharmacies countrywide.

To get a prescription, you must first consult your ophthalmologist to know whether you qualify for the presbyopia medication.

Fda Approves Eye Drops For Treatment Of Presbyopia

Allergan developed VUITY, which it noted is the first and only FDA-approved eye drop to treat presbyopia.

Allergan, an AbbVie company, announced FDA approval of pilocarpine HCl ophthalmic solution 1.25% for the treatment of presbyopia, commonly known as age-related blurry near vision, in adults.

According to the company, pilocarpine HCl ophthalmic solution 1.25% is the first and only FDA-approved eye drop to treat this common and progressive eye condition that affects 128 million Americans, nearly half of the US adult population.

Michael Severino, MD, vice chairman and president of AbbVie, noted that most adults cope with presbyopia, or difficulty with near vision, as they age.

Can Atropine Be Used In Combination With Other Myopia Control Strategies

It is generally safe to combine atropine for myopia control with specialized glasses or contact lenses for myopia control. You should begin one strategy first, and when your eye doctor thinks it is appropriate, they can start the additional strategy.

Side effects of atropine eye drops include:

Other alternatives for improving near vision, some of which are permanent, include:

Vuity Efficacy And Safety

In studies, VUITY improved near vision without compromising distance vision. This was measured by the proportion of patients achieving a 3-line gain or more reading a near vision eye chart without losing more than 1 line in a distance vision eye chart at Day 30, 3 hours after dosing.

How And When Vuity Works

Optive Advanced Eye Drops 15ml

The eyes pupil naturally constricts in response to light. When it does, a persons depth of field expands, allowing the eye to focus at expanded distances. The result: improved near and intermediate distance vision.

Pilocarpine, the active ingredient in Vuity, contracts the ciliary muscles that help control pupil size. Pilocarpine drops were the primary treatment for glaucoma until the 1980s and are occasionally still used today.

The once-daily drops work in 15 minutes or less, and can last up to 6 hours for near vision and 10 hours for intermediate vision .

The FDA based its approval of Vuity on findings from two phase 3 clinical trials: GEMINI 1 and GEMINI 2.

In both trials, those who took the Vuity drops were significantly more likely to make out three or more additional, smaller-type-sized lines on a reading chart compared with the placebo group.
