Davis Vision has been chosen to serve as our partner for routine vision, glasses and contacts beginning January 1, 2021 for BlueCross BlueShield Commercial and Medicare Advantage patients. Because EyeMed will no longer be our vision partner beginning on January 1, we encourage you to join the Davis Vision provider network so you may continue to serve your BlueCross BlueShield patients.

Through this partnership, we will be able to offer more comprehensive coverage and ancillary offerings. Routine vision benefits may vary by plan for your BlueCross BlueShield patients. Coverage for problem eye services is considered a medical benefit and will not be managed by Davis Vision.

Pediatric Exchange Vision Coverage

One exception to the Davis Vision arrangement is for pediatric patients who are covered by our Individual and Small Group plans purchased through the New York State of Health marketplace or directly through BlueCross BlueShield. These patients will need to see BlueCross BlueShield network vision providers for routine eye exams, and eye care accessories as these benefits are embedded in the medical benefit.

Davis Vision, a national administrator of routine vision care programs, provides benefits under the Vision Plan. Each enrolled person may receive an eye exam, one pair of eyeglasses or contact lenses, and frames once each calendar year. There is a monthly rate for enrollment in the plan and there may be an additional copay when you receive services.

You can receive service from providers who participate with Davis Vision or from non-participating providers. The greatest cost savings are with participating providers.

What Is On My Member Id Card

There is a lot of important information on your member ID card. Heres what it all means.

  • Member Name: In most cases, this will only show the subscribers name, not the covered dependents name. The doctor and hospital will accept this card for you and your covered dependents.
  • Member ID Number: Your member ID number is made up of a series of letters and numbers. It will be the same for everyone covered under your Horizon plan and will remain as your ID number for as long as you are the subscriber of the health plan. Generally, the numbers after the 3HZN are what doctors and hospitals will need from you. The prefix is used by Horizon to route claims for processing.
  • Coverage Verification Data: Doctors and hospitals need this information to check eligibility and submit claims. They can also sign in to the provider website to verify your coverage.
  • Primary Care Physician name, if a PCP is required by your plan.
  • Name of your Horizon Plan: You will need to know this when looking for participating doctors and hospitals in the Doctor & Hospital Finder.
  • Copayment Information: This will show only if your plan includes a copayment. The copayment is how much you pay, if applicable, each time you get care, treatment or services. The amount may vary depending on where you receive care . Your copayment is not your coinsurance amount. Some member ID cards will also show your coinsurance and deductible amount, if this applies to your plan.
